Please arrive 15 - 20 minutes early to fill out the new patient forms and health questionnaire.
You may also download and fill out the forms prior to your visit.
Please remember to bring the following:
- Driver’s License or State ID
- Referral if required by your insurer
- Insurance information
- Copies of your operation records, medical records, imaging test results
(X-rays, MRI, CT scans, etc.), etc. from your previous doctor visits.
In the event that you need to cancel your appointment, please provide us with a 24 hours notice.
Your consideration in this matter will allow us to accommodate other patients in need of an appointment.
Insurance and Billing
We strive to provide all of our patients with affordable medical care. In order to do so, we request that payment arrangements be made at the time of services rendered. We accept cash and all major credit cards (Visa, MasterCard, American Express, Discover) for the convenience of our patients. We will also bill your insurance company and assist in handling claims in every way we can, but responsibility for payment ultimately falls to the patient.
Our main focus is to provide our patients with the appropriate medical care for their individual needs. Our billing office will work with you regarding any special needs or concerns you may have.
